There are a bunch of companies in the US that do them. Voler (www.voler.com) and Bikeatac (www.bikeatac.com) are the two that come to mind first because I've been looking into them for the Cancer Crusader's custom jerseys.

If you do a goole search for "custom cycling jerseys" a lot will come up...some have pretty steep minimums, while others will do as few as one, but the cost is a lot higher.

I've got the process started for a "teaser" jersey for Cancer Crusaders. I'll post some pics when I have it in hand. :)
